The twilight years of one’s life can be daunting. Understanding that death is natural and inevitable helps in planning for end-of-life care. The Access TLC Foundation helps families in Southern California to make end of life decisions.
This type of care aims to ensure comfort, choice, and quality of life. It recognizes and respects individuality and psychosocial needs of patients. It also aims to reduce unnecessary and burdensome treatments in order to reduce pain and stress.
A growing number of people need specialized medical attention in their final days. Approximately 25% of all U.S. deaths take place in the long-term care setting, and this figure is projected to rise to 40% by the year 2040.
A study showed that elderly heart-failure patients who received specialized nursing care during their hospital stay and at home had a better quality of life and faced fewer hospital readmissions.
Affordability becomes an issue for low-income families who do not have the means to provide their loved ones with the kind of professional treatment and care they need and deserve.
Access TLC Foundation: Compassionate End of Life Care
Access TLC makes such treatments affordable and thus more easily accessible to these families. Treatments include a combination of hospice, caregiving and healthcare.
Patients who cannot look after themselves and meet the criteria for eligibility can qualify for health care at home. This is to ensure that they feel as comfortable as possible in their challenging condition. Some of the criteria include an intermittent need for nursing care, physical therapy, and pathology service.
Access TLC provides critical services such as assessment, medication, rehabilitation, and post-op care to eligible patients. It also provides hospice care for patients with terminal illnesses which is fully covered by Medicare. In addition to high quality medical treatment, Access TLC also provides emotional support to patients and their loved ones.
Palliative care as active holistic care is offered to patients with advanced progressive illnesses. According to the NHS, the purpose of Palliative care is to make patients as comfortable as possible through pain-management and the alleviation of distressing symptoms. It can also be viewed as a form of medical support intended to lessen psychological pain through social and spiritual care.
Bereavement counseling is also given with the purpose of helping families in situations of painful loss.
Access TLC’s Mission
This organization is dedicated to providing high quality services to help people experience a better existence. Its mission is to provide Tender Loving Care to its patients with sincerity, commitment and excellence. Their goal is to offer quality care and programs that surpass expectations and make a real difference in the patient’s health.
Transport services are provided around the clock to patients that require access to stretches or wheelchair. For patients who are unable to get around on their own, their transportation needs are covered in the programs. This makes it convenient and easier for them to have constant access to all their medical appointments or other needs.
A Second Home for the Elderly
The 5000 square foot Access TLC Health Care Center facility in Moorpark, California, provides designated nursing services for patients who need round the clock health care. These include patients suffering from the latter stages of serious illnesses and require dedicated hospice care.
Recreational Activities for the Elderly
A large volume of literature has investigated the relationship between cognition and social activities or social engagement among the elderly. These studies confirm a positive relationship between the two.
Access TLC incorporates recreational activities to ensure patients have a source of companionship as many are physically unable to leave their homes. Even a brief encounter with a volunteer or friendly person can have a tremendous uplifting affect.
Access TLC is also dedicated to promoting education on health care and feature events such as film screenings to provide insights and knowledge on hospice care.
This is crucial to educating the general public on what types of services are available for certain needs including hospice care for terminal patients. They also benefit physicians who can receive valuable feedback from families in order to improve and tailor the types of services that they can provide to clients in need of their expertise.
Patient grants are also available to assist in providing necessary funds to access end of life care and are approved on a case by case basis. Grants can help provide caregiver services as well as medication or aid with burial processes.
Your Help Can Extend Lives
Early admission and access to palliative care can extend a terminal patient’s life. Patients with cancer or advanced heart and lung illnesses represent a large proportion of those under hospice care. Access TLC has a Palliative care team comprised of different healthcare professionals that co-ordinate and care for people with an incurable illness.
Many low-income families struggle to give their elder counterparts the professional care need. This is where Access TLC Foundation comes in and ensures the provision of optimal care for end of life patients who may otherwise not have access to premium hospice or health care.
Volunteers Are Welcome
A dedicated group of volunteers undertake a large portion of operations, and volunteers are always welcomed. Qualifying hospice volunteers can serve an important role alongside end of life patients, equipped with training and support to provide a source of comfort and feedback to patients in their final stages.
The Foundation also receives support from a dedicate group of volunteers in fundraising activities. Access TLC currently hosts an annual Kentucky Derby Gala fundraiser at the Camarillo Ranch which provides dining options alongside entertainment such as horseracing as well as auctions.
